NEW YORK (AP) — The Food and Drug Administration has approved a generic version of a popular arthritis medication called Celebrex to reduce the risk of heart attacks and strokes in patients over 65.
The drug, called Vioxx, belongs to a class of drugs called Cox-2 inhibitors. It is the second most popular arthritis drug in the world and is used to treat both rheumatoid arthritis and osteoarthritis.
The FDA said the agency was also reviewing whether Vioxx should be marketed as an over-the-counter arthritis medicine.
The agency is also considering whether it should be the first prescription drug in a class of drugs called nonsteroidal anti-inflammatory drugs (NSAIDs), or nonsteroidal anti-inflammatory drugs (NSAIDs-NSAIDs), or nonsteroidal anti-coagulants. In addition, the FDA said it is studying how the generic drug could be marketed to increase the safety of the drugs.
Vioxx and Celebrex both are approved for uses in adults. But the drug has been linked to serious side effects in at least 2,000 people over the past decade. More than 500,000 people have been hospitalized due to stomach ulcers, bleeding or other serious conditions.
The Food and Drug Administration said Celebrex is not approved for use in pediatric patients.
The FDA has said there's no evidence that it's safe and effective for treating children and adolescents with arthritis.
The agency also said Vioxx may cause serious side effects in patients under 18.
The FDA said the risk is lower for people who are taking a class of drugs called Cox-2 inhibitors, which include a group of arthritis drugs called cyclo-oxygenase (COX) inhibitors. In patients with osteoarthritis, Celebrex can reduce the amount of pain-relieving chemicals in the joints that may cause arthritis.
The FDA also said that Vioxx is not approved for use in people with severe liver disease, who also have problems with the kidneys.
Vioxx and Celebrex are not approved for use in pediatric patients.
The agency said the FDA is reviewing the safety and effectiveness of these drugs in children and adolescents with osteoarthritis and rheumatoid arthritis.
The agency is also reviewing whether other drugs for pain relief, including naproxen, which is sold as Aleve, are safe and effective for children and adolescents with arthritis.
The FDA said it was also considering whether Vioxx could be used in people with a history of stomach ulcers and bleeding, in whom other drugs for pain and inflammation are unlikely to be effective.
The FDA said the drug is not approved for use in children or adolescents.

It belongs to the class of drugs known as non-steroidal anti-inflammatory drugs (NSAIDs). These drugs work by blocking the production of prostaglandins which cause pain and inflammation. The European Medicines Agency (EMA) warned that the COX-2 antagonist Celebrex (celecoxib) caused a decrease in the clearance of COX-2-bound drugs in patients receiving daily doses of up to 3 times the maximum daily dose (MDD). However, the FDA advised that Celebrex is not recommended for use by patients who have not achieved a satisfactory response to COX-2-selective inhibitors (C icing, Vectrin, Vioxx, Plavix, etc). The FDA has not authorized Celebrex in patients receiving daily doses greater than 3 times the maximum daily dose (MDD).
The EMA’s FDA’s guidance states that “selective COX-2 inhibitors are safe to use with caution in patients who are currently or may be taking multiple daily doses of C icing.” In particular, the FDA’s guidance states that, when used in conjunction with a COX-2-selective inhibitor, a patient with severe COX-2-related gastrointestinal side effects must not use Celebrex.
The FDA’s guidance is based on studies that have found the risk of COX-2-related gastrointestinal side effects with daily doses greater than 3 times the maximum daily dose (MDD) to be greater than 40%.
For Celebrex, the FDA warns that it is important for patients to complete a full questionnaire prior to taking Celebrex. Patients must complete the full questionnaire to obtain the maximum COX-2-selective inhibitor, and then take the medication as prescribed.
